Change in Transportation Form

  • Please use this form to report temporary changes in transportation plans to the London Towne main office up until 2:00 p.m. on the day of the change. 
  • If the change in transportation involves another student, families of BOTH students must fill out forms.
  • If you prefer to not use this form, please call the main office at 703-227-5454. 
  • Permanent changes MUST be reported by calling the main office at 703-227-5454 AND completing a TR-52 form, which can be obtained from the main office.

This attendance form is to be submitted by the parent/legal guardian only.*
Please enter the date of the temporary transportation change. Transportation form changes must be reported by 2:00 p.m. on the date of the change. For changes being reported after 2:00 p.m., please contact the attendance 703-227-5454.
